This supplement provides a numerical index of AKW Forms currently in effect at Alaska Wing Headquarters CAP. This publication applies to units and members of the Alaska Wing CAP. Astericks * denotes new/or revised forms.

                2.1. Requisition on CAP Form 8 (one copy) by mail to:

2.2. Semiannual Requisitions. Every 6 months, units should inspect their blank forms and requisition current

    copies needed until the next semiannual requisition (refer. CAPR 5-4, par.2).

2.3. Out of Cycle Requisitions. Out of cycle requisitions should be held to an absolute minimum. However

    requests may be submitted when required.

2.4. Local reproduction of AKW forms is authorized and encouraged.

All previous AKW Forms not listed in this supplement are rescinded
