SUBJECT: Alaska Wing Policy No. 5

                   Form 5 Recommendation Sign Off

1. This policy replaces the Alaska Wing/DOV letter on the same subject, dated 10 February 1999, and is intended to convert the established guidance into the Wing’s standard numbered policy format.

2. CAP pilots receiving a Form 5 check ride from a Check Pilot must first have a sign off from an instructor pilot, recommending him/her for the Form 5 check ride. This directive applies only to the initial Form 5 check ride in a corporate aircraft.

            a. The instructor sign off must be in writing on the CAP Form 5, which is used by

               the Check Pilot.

            b. The instructor who does the recommendation sign off cannot also conduct the

                Form 5 check ride.

3.The Wing Office of Primary Responsibility (OPR) for this policy is Standards and Evaluation (StanEval). Contact the OPR if you have any questions or recommendations concerning this policy.
