SICK LEAVE BANK

ENROLLMENT FORM

The Juneau Education Association has negotiated an employment benefit for certificated personnel covered by our Negotiated Agreement. Our Negotiated Agreement states that "In order for a teacher to draw from the Sick Leave Bank the teacher must have contributed at least one (1) day to the Bank during the first thirty days after commencement of service."

A Member of the Sick Leave Bank may receive up to, but not more than, twice (2X) the number of sick leave days he/she has credited to him/her on the first day of the school year.

Please complete one option on the form during the first 30 days of your service. Make two copies. Send the original to Deedie Sorensen at Mendenhall River Community School via the school mail. Keep the other copy for your own files. Deedie will make a copy for JEA and send the original to Payroll.

I wish to participate in the JEA Sick Leave Bank by donating one (1) day, if and when the Sick Leave Bank balance falls below 100 days, in accordance with the provisions in the Negotiated Agreement.
